Required Forms and Regulations

Below please find the Summer Youth Program required forms needed to be on file prior to the start date of a student’s first workshop.

  • Health Forms – After students register for one of the Summer Youth Programs, they are required to complete and submit their health forms to the Center for Continuing and Professional Education office via fax, electronic mail at youthprograms@wpunj.edu and/or post mail prior to the first day of class.  A physical by the primary doctor is not required; however, the health form needs to be reviewed and signed by the doctor’s office. 
  • Release Forms –Please review, answer the appropriate questions, sign the release form and submit it to the Center for Continuing and Professional Education office via fax, electronic mail to Rosemarie Zimmerer or Rosie Williams-Hopkins at Youthprograms@wpunj.edu and/or post mail prior to the first day of class.
  • Rules and Regulations – Please take the time to review and discuss the Summer Youth Programs’ rules and regulations with the student participating in the program.  The following items will be addressed: (1) William Paterson University’s Youth Program has a Zero Tolerance Act for commuters and resident students; (2) computing policies and guidelines of misuse of Information Technology resource privileges; and (3) residence halls rules and regulations.
