MLTSS for Children: This is the only Medicaid waiver program available to children The child’s medical needs must meet the nursing home level of care To qualify, the child will need to have very significant medical needs Parental income and assets are not considered For children birth through 20 years old contact the Division of Disabilities Services (DDS) at 1-888-285-3036 (press 2 after prompt and then press 1 after next prompt) to speak with an Information and Referral Specialist. How to apply for an adult 21 years of age and older: Contact your County Aging and Disability Resource Center (ADRC) or your County Welfare Agency (Board of Social Services) to initiate an assessment for MLTSS eligibility. How to apply if a child or adult currently has Medicaid: Both children and adults who currently have Medicaid should contact their Managed Care Organization to request an assessment for MLTSS eligibility.
