Workers Compensation There are occasions in which an employee may sustain an injury while performing their job duties. We want to ensure that such injuries are treated promptly and properly. The State of New Jersey has selected Horizon Casualty Services to provide a managed care program to administer the University’s Workers’ Compensation claims. What to Do If You Are Injured Emergency Work-Related Injuries If you have suffered a life-threatening injury or in the case of an emergency, contact Campus Police immediately at 973-720-2300 and Campus Police will dispatch Emergency Medical Services (EMS) for treatment and transportation to the nearest hospital. Immediately notify your direct supervisor and the Benefits Office in the Division of Human Resources at 973-720-2124 or e-mail EmployeeBenefits@wpunj.edu and report injury. Do not delay obtaining medical care to do so. Complete and submit RM-2 form within 48 hours to the Benefits department at EmployeeBenefits@wpunj.edu. or upload the RM2 form in WP-Connect under “My Document Upload.” The supervisor and employee should follow instructions for Accident Report Complete and submit the Outside Employment Form within 48 hours to the Benefits department at EmployeeBenefits@wpunj.edu. or upload the Outside Employment Form WP-Connect under “My Document Upload.” Provide any medical documentation from the hospital along with your RM2 report. Non-Emergency Work-Related Injuries Immediately notify your direct supervisor and the Benefits Office in the Division of Human Resources at 973-720-2124 or e-mail EmployeeBenefits@wpunj.edu and report injury. Once HR has been notified that the employee is seeking medical treatment an authorization form will be sent to Concentra. Complete and submit RM-2 form within 48 hours to the Benefits department at EmployeeBenefits@wpunj.edu or upload the RM2 form in WP-Connect under “My Document Upload.” The supervisor and employee should follow instructions for Accident Report RM2. Complete and submit the Outside Employment Form within 48 hours to the Benefits department at EmployeeBenefits@wpunj.edu. or upload the Outside Employment Form WP-Connect under “My Document Upload.” Accident Report In order for you to be considered for Workers’ Compensation Benefits, a State of N.J. Accident Report (Form RM-2) must be completed by the employee and signed by both employee and their supervisor. The employee must submit the form to the Benefits Office within 48 hours after the injury occurred in accordance with New Jersey State regulations governing Workers’ Compensation benefits. The Benefits Office will send the signed RM2 form to the NJ Treasury Division of Risk Management. A police report may Not be used as a substitute for the RM2 form. Treatment Facilities An injured employee must follow the reporting procedures set forth and seek treatment in accordance with this managed care program. Employees who do not comply with these procedures will be responsible for any unauthorized medical bills. Please note: Unauthorized Medical Treatment ‐ Employees should not seek treatment from their primary‐care physician. Such treatment can be deemed unauthorized, and the employee will be responsible for paying their medical bills. Prior to any absence due to a work-related injury being approved, the employee must have followed the above required procedures. It is the employee’s responsibility to provide the proper medical documentation from an approved physician supporting the absence to the Benefits Office in timely fashion. In the rare instance in which there is reason to believe that work-related sick leave is being abused, the University or its designee may choose to conduct an investigation. If the University concludes that there has been abuse, disciplinary action, up to and including termination will be taken. In addition, where appropriate, the matter will be referred to the State of New Jersey’s Attorney General Office for legal action.
