Complaint/Incident Reporting Forms

William Paterson University is committed to providing students with a positive learning environment and to reviewing and responding to student complaints and concerns appropriately. Below is a list of the various reporting options and forms to use to submit those reports.

To report a general student complaint:

The complaints that should be reported via this link are limited to those not already covered by other existing policies and procedures, and that meet the definition of a complaint, as outlined in the General Student Complaint policy.

Complaints will be responded to by the Dean of Students or an appropriate designee.

To report allegations of bias related incidents:

These reports will be responded to by a member of the Bias Prevention & Education Team (BPET). More information about BPET can be found here.

To report allegations of violations of the Student Code of Conduct:

These reports will be responded to by the Director of Student Conduct. The Student Code of Conduct can be found here.

To make an anonymous report to the University Police Department:

Students are permitted to make anonymous reports of a crime, or other incidents of concern, to the University Police Department, but please do not use this form to report an incident or crime that requires immediate response from the University Police. In these situations, please call 911 or 973.720.2300.

To report allegations of sexual harassment and misconduct:

These reports will be responded to by the Director of Institutional Equity and Compliance/Title IX Coordinator. More information about Title IX can be found here.
