Cost Share Request Form OSP Cost sharing indicates the use of institutional funds to supplement a sponsor's support of a project. This form should be used for cost
sharing commitments in proposal budgets and must be submitted to the Office of Sponsored Programs (OSP)

Prior Approval & Waiver Request

Form Office Description
IDC Waiver Form (F&A Waiver) OSP

Form seeking approval for full or partial waivers of the indirect costs (F&A) normally incurred by sponsored projects.
Course Release (Reassigned Time) Request Form OSP (Proposal stage) Form seeking departmental approval for effort allocation required for a proposed sponsored projects.
Award Acceptance Form OSP (Award stage) Form seeking departmental approval for effort allocation required for sponsored projects.

Compliance (COI, IRB, IACUC)

Form Office Description
IRB Application Form OSP WP Faculty, Staff or Doctoral-level student intending to conduct research involving human subjects, need to complete this form to submit your protocol for review by the WP IRB.
IRB Decision Tree OSP Tool to determine if your project needs to be reviewed by the IRB.  There is even an option for projects using secondary data.
Conflict of Interest and Commitment Disclosures (COIC) OSP

In accordance with WPUNJ Functional Policy 02-15-80-00-15 on Conflicts of Interest and Commitments, the University enforces compliance requirements related to disclosures of conflicts of interest, commitment and review at the time of proposal and award and for IRB applications submitted by faculty, staff and doctoral students.  

Note: Investigators with on-going, funded research are required to complete a Conflict of Interest and Commitment (COIC) Disclosure annually, and also within 30 days of any change to the last disclosure.
